Please do not come to your appointment if any of the following apply:

• You have tested positive for Covid in the last 14 days

• You are waiting for a test or a test result

• You have any symptoms of a new continuous cough, loss of taste or smell or a high temperature, runny nose and headache

• You are living with anyone with symptoms

• You have been contacted by the NHS Track & Trace and told to isolate
